Work with the Interdisciplinary Team in developing a comprehensive resident assessment and care plan for each resident based on their condition and pertinent medical data to determine any need for special assessment, activities, or a need to amend the admission assessment. Ensure that each portion of the assessment is completed, signed and dated to certify its completion. Develop, implement, and maintain an ongoing quality assurance program for resident assessment/care plans. -Attend and participate in annual facility in-services and continuing education programs designed to help keep abreast of changes, as well as to maintain RAC-CT certification. -
